If you are looking a format to put your salary information into a printed form to be used as salary slip, then you can use this salary slip template. It has format for hourly rate employees. It is just a one form template where you can type all of your data and print in A5 paper size. No calculator function.

As you can see from screenshot above, you can edit header part with your company information as well as its address.
In the second part, you can type your name or your employee’s name, ID, SSN, pay period (weekly, biweekly, monthly or any period) and pay date. You can delete information that you don’t want to put into your pay slip or edit or add more lines.
In salary part, you can type categories for wages. Since it is created for hourly rate employees, you can see hour column inside salary table. You can type number of hours for categories that are based on hours. You can type total amount for categories that are not based on hours.
In YTD column, you can entry total amount up to particular pay period.
And it the last part, you can fill your employee’s bank information and write total amount in words. Simple SUM formula will sum total amount automatically.
